Truck Driver Physicals for Patients and Non-Patients—Effective 05-21-14, the Federal Motor Carrier Safety Administration (FMCSA) will require Physicians and Health Care Professionals to be certified for DOT Physicals. Dr. Adam Houg and Julie Monenter, PA are certified providers and are prepared to meet the new requirements. For non-patients these physicals cannot be incorporated with your yearly wellness and must be paid at time of service. Feel free to contact our office to schedule your DOT Physical.
Billing
Our billing department staff is available during regular office hours to assist you in answering your questions regarding your account balance. If you are unable to pay your balance in full, we can arrange a budget plan for you. We accept cash, check, credit card (Visa, MasterCard, Discover) and now accepting CareCredit. For your convenience, we offer the option of making your credit card payments by phone. Co pays for the office visits are expected at time of check in.
